Is TTFB taking place only the first time a URL is being visited?

Not really. Team leads, development operations engineers and network teams from a hosting service company having the best Los Angeles Dedicated Servers state that TTFB is not necessarily taking place the first time a URL is visited. It is also taking place each time access to content is requested.
